ATTENTION:   Membership Renewal Statements (MRS) are mailed in June annually.  You can use this form to request a replacement MRS which will be emailed to you in PDF format.  Only Commanders and Adjutants can request MRS statements for others.  If you don't know your SCV number, please contact the DIVISION ADJUTANT for assistance.    Once we receive your request,  please allow 72 business hours for a reply.  We will verify your information, and we will be in contact.  Deo Vindice!
